Once your provider approves you for a med card and you pay the state’s $8 fee, you’ll receive your card immediately.

Meet with a pharmacist*
For first-time cardholders, Utah requires a one-time pharmacist consult which we offer for free through our partner pharmacy next door. Our team will book this automatically for you after your provider appointment.

Know
Requirements
You must be a Utah resident, qualify for a med card through one of Utah’s qualifying conditions, and provide medical documentation of your condition (if applicable).
What to Bring
Valid, non-expired, government-issued identification (driver’s license, passport, military ID, or state ID)
Credit, debit, or cash to pay the appointment cost of $70
Debit or credit card to pay the state fee of $8 online
Internet-capable device to access your profile on evs.utah.gov
Medical documentation of your qualifying condition (if applicable). Not all conditions require documentation—if your qualifying condition is acute pain or chronic pain documentation is not required.

We’ve capped our provider appointment price at $70. There is also a mandatory state fee of $8 to receive your card after you’ve met with the provider. The med card will be valid for 1 year from date of issue.

Times vary, but most patients can plan for about 30-45 minutes in total.
New patients are required by the state to complete a one-time pharmacist consultation before making their first purchase, which can take an additional 10-15 minutes.
Renewing patients are not required to meet with a pharmacist, so the appointment may be as short as 15 minutes.
