Get your med card 5 days a week, always $70 or less. Walk-ins welcome. Monday–Thursday 11am-4pm, Friday 1-6pm.

At your appointment, the UTMC team will help you complete all the steps required to get your med card. Most patients are able to receive their card and make a purchase same-day.

You will:

✓ Meet with a provider to receive approval

✓ Complete all state requirements

✓ Receive your med card so you can shop same-day

✓ Get access to exclusive deals & discounts with our partner pharmacy

What to bring:

  • Identification (passport, driver’s license, military ID, or other form of valid government-issued ID)

  • A device to access evs.utah.gov (phone, tablet, or laptop)

  • A form of payment for the state’s $8 fee—debit or credit accepted

  • A form of payment for the appointment cost of $70—cash, debit, or credit accepted
